What you are signing

The full text is in the form itself. This is the plain summary, not a replacement for reading it.

Permission to coach

You confirm you are the parent or legal guardian, and you give permission for your child to take golf instruction with Colin Coughlin. Golf involves ordinary physical risk — clubs, balls, equipment, movement, weather — and the form says so plainly.

How lessons are run

Lessons with minors are held in an open, observable setting wherever reasonably possible, and you are welcome to stay and watch. CC Golf Lab does not provide transportation. You choose whether Colin communicates only with you, or with your child directly about lessons and scheduling with you kept in the loop.

Emergency information

You can list allergies, medical considerations, or physical limits Colin should know about, plus an emergency contact. If something happens and you cannot be reached, you authorize Colin to call emergency services and take reasonable steps to protect your child.

That information is entered on the signing page and stays with the signed document. It is not collected by this website — the form above only asks where to send your link.

Video, and the part that is optional

These are two separate decisions and the form keeps them separate. The first is instructional recording — filming a swing for private coaching use, to analyze technique and track progress. The second is permission to use photos or video publicly, on the website, social media, or in advertising.

The public one is optional and saying no to it changes nothing about the coaching your child receives. Where it is authorized, your child’s full name is not intentionally published alongside their image.
